Hypothyroidism

     Hypothyroidism (underactive thyroid or impaired production of thyroid hormone) is a condition of insufficient thyroid hormone in the body. It develops when the thyroid gland fails to produce as much as thyroxine (T4) and triiodothyroxine (T3) as the body needs. Since the main purpose of hormones produced by thyroid gland is to "run the body's metabolism," it is understandable that people with this hypothyroidism or underactive thyroid would have symptoms associated with a slow metabolism causing damage to organs and tissues of every part of the body and leading to life threatening complications.

     Hypothyroidism (underactive thyroid or impaired production of thyroid hormone) is one of the most common chronic diseases in the developed countries like United States, United Kingdom and so on. And it is now getting very common in developing countries like India, China and so on.

    Although this condition is believed to affect as many as 11 million adults and children, but 2 out of every 3 people with hypothyroidism may not know they have the disease. Symptoms may not appear until years after the thyroid has stopped functioning and they are often mistaken for signs of other illnesses, menopause and aging.

      Hypothyroidism is at least twice as common in women as it is in men. Although it is most common in women who are middle aged or older, the disease can occur at any age.


Causes of Hypothyroidism
(Underactive Thyroid or Impaired Production of Thyroid Hormone)
• Chronic Autoimmune Thyroiditis (Hashimoto's thyroiditis) : A form of thyroid inflammation caused by damage to thyroid tissue.
• Autoimmune diseases : You are at more risk for hypothyroidism if you have diseases like rheumatoid arthritis, vitiligo, addison's disease, diabetes and so on.
• Surgery : Removal of a partial or all of thyroid is another cause.
• Radioactive iodine therapy.
Secondary Hypothyroidism : Disorders of pituitary and hypothalamus leading to thyroid hormone deficiency in addition to other hormonal imbalances.


Symptoms of hypothyroidism

(underactive thyroid or impaired production of thyroid hormone) mainly depend upon the severity of the hormone deficiency and its duration. For most patients, the symptoms are mild and can often be confused with other problems. The symptoms may include :

GOITER

Chronic enlargement of the thyroid gland not due to neoplasm is called as goiter.
• Endemic goiter-Areas where > 5% of children of 6-12 years of age have goiter, very common in China and central Africa.
• Sporadic goiter -Areas where < 5% of children 6-12 years of age have goiter. Multinodular goiter in sporadic areas often denotes the presence of multiple nodules rather than gross gland enlargement.
• Familial


Etiology:
1. Hashimoto’s thyroiditis
• Early stages only, late stages show atrophic changes
• May present with hypo, hyper or euthyroid states


Graves’ disease – Due to chronic stimulation of TSH receptor


1. Diet –  (cabbage, turnips, cauliflower, broccoli), Cassava, Lithium prevents release of hormone, causes goiter in 6% of chronic users
2. Neoplasm
3. Chronic Iodine excess – Iodine excess leads to increased colloid formation and can prevent hormone release. If a patient does not develop iodine leak, excess iodine can lead to goiter.


THYROID NODULES
• Prevalence
– Palpable: 4-7%
– Non-Palpable: >50%
– Cancer in nodules: 5%
• Women affected more than men
• Most subjects are euthyroid and/or asymptomatic
• Prevalence is less than 1% with thyrotoxicosis


SOLITARY THYROID NODULE



• More likely to be malignant in men, patients over 60 and patients with a h/o head or neck irradiation are at more risk.
• No growth for years almost always indicative of benign nodule as it is not a nodule that appears suddenly (likely a cyst or adenoma hemorrhage)
• Malignant nodule develops in weeks to months.
• Virtually all patients with thyroid carcinoma are euthyroid as are those with benign nodules. Nodule of >1.5 cms., are usually detectable on examination and are confirmed in the Ultrasonography. Lifetime risk for developing a nodule is 5-10%. Studies show 50% of people during autopsy have either single or multiple nodules. 5-10% of clinically detectable hypofunctioning (cold) nodules can be malignant. The laboratory/imaging techniques used are: TSH, Calcitonin, Ultrasound, FNB for characterization of Nodules, Nuclear Scan to see whether nodule is “Hot” or “Cold.” If FNB is suggestive of malignancy then surgery is advised, and if it is suspicious or negative then a follow up of few months is given to the patient with repeat investigations. In case there are indeterminant reports then FNB is repeated, if still indeterminant, surgery is recommended.

THYROTOXICOSIS

Definition – refers to clinical syndrome of hyper metabolism resulting from increased serum concentration of T4 and/or T3, regardless of cause. Not synonymous with hyperthyroidism. Hyperthyroidism – increased thyroid hormone biosynthesis and secretion by the thyroid gland.


Symptoms – nervousness, fatigue, weakness, increased perspiration, heat intolerance, tremor, hyperactivity, palpitation, change in appetite, weight change, menstrual irregularities.


Signs - hyperactivity, tachycardia, atrial arrhythmia, systolic hypertension, stare, eyelid retraction, tremor, hyperreflexia, muscle weakness.


Older patients - have fewer signs and symptoms of sympathetic activation and more cardiovascular dysfunction.


GRAVES’ DISEASE



It is the most common cause of thyrotoxicosis in the industrialized world. It is an autoimmune condition with anti-TSHr antibodies. Onset of disease may be related to severe stress which alters the immune response.


Diagnosis:


TSH, T4, T3 to establish toxicosis


CANCER OR TUMOR

Healthy cells in our bodies are always dividing and replacing old cells in a very controlled fashion. According to scientists, about 10 million cells in a human body die and are replaced every second. When this process of controlled multiplication of cells become uncontrolled and does not stop it leads to cancer. This cluster of cells which is dividing uncontrollably is called a Tumor. Tumors can be of 2 types:

• Benign or non cancerous tumors: They stop growing, do not spead to other parts of the body and do not give rise to new tumors.

• Malignant or cancerous tumors: They grow and croud out healthy cells, interfere with functions of the body and suck nutrients from body tissues. They also spead and form new tumors in other parts of the body. This process of spreading of tumor to other parts of the body is called metastasis.

Malignant or cancerous tumors are further divided into 4 types:


• Carcinomas: These are the most commonest and usually affect breasts, skin, lungs, pancreas and other glands and organs.
• Lymphomas: These are cancers affecting the lymphatic system.
• Leukemias: These are cancers of the blood; they do not form solid tumors.
• Sarcomas: These are rare and involve bone, muscle or cartilage.


Causes of Cancer or Tumor?

The basic cause of all cancers is change or mutation in the nucleus of a cell which leads to constant uncontrolled cell division. In a healthy human being the body's immune system can recognize cells which have turned cancerous and destroy them before they can multiply. Cancer occurs when some mutant cells evade detection by the immune system and survive.

There are many factors know as risk factors which start or promote this process of constant uncontrolled cell division in a cell:
• Inherited or familial predisposition.
• Tobacco smoking is linked to cancer of the lung, head and neck area, stomach, kidney, bladder, pancreas, cervix and leukemia.
• Excessive exposure to sun light is linked to cancer of skin.

• Excessive consumption of fat, alcohol, foods that have been pickeled, smoked, charred or cured lead to cancers of gastrointestinal tract, breast, uterus and prostate. Lack of dietary fibre and antioxidants is also a contributory factor.

• Various substances, chemicals and gases found in water, air, foods, tobacco smoke, pesticides, paints, cleaning products are carcinogens.
• Radioactive waste, x-rays, nuclear radiation are also potent risk factors of cancer.
• Certain viruses such as Epstein-Barr, hepatitis-B, HIV and papilloma are also contributory to cancer or tumor.


     All the above factors contribute to cancer causation but cancer is not caused by one single factor alone. Cancer occurs when there is a mix of age, inherited disposition, general health and carcinogenic exposure.


Seven Early Signs of Cancer
It is important to diagnose cancer as early as possible because it makes a huge difference in the treatment and its outcome.
To remember these early signs (according to American Cancer Society) we should think of word CAUTION.
• Change in bladder or bowel habits.
• A sore that does not heal.
• Unusual discharge or bleeding.
• Thickening or lump in the breast or elsewhere.
• Indigestion or difficulty in swallowing.
• Obvious change in a wart or mole.
• Nagging cough or hoarseness.


Symptoms of Cancer or Tumor
Early diagnosis and treatment of cancer is very important because it offers a better chance of cure.

In its initial stage a cancer or a tumor does not exhibit any signs or symptoms. But as a cancer or tumor grows in size it becomes easy to detect it.

As a cancer or tumor grows it may press upon a nerve and cause pain. It may also cause bleeding by penetrating blood vessels. It may also interfere with body functions by causing obstruction.

Homeopathy has not given the seriousness it deserves from medical authorities and governmental agencies, and I feel when suddenly its worth will be discovered the whole world will turn towards it with an unprecedented hysteria expecting much more that it can deliver. Homeopathy is very difficult in its learning and practicing, and requires a kind of dedication on the part of the physician who undertakes to learn it properly and practice it professionally. Insurance companies together with governments should pay more attention and money in to a really good education that doctors are needed in order to show the results.

The law of similars: ‘similia similibus curentur.’

In order to ascertain the therapeutic qualities of pharmaceutical substances, Hahnemann carried out provings of the remedies on healthy persons of both sexes in increased dosages, though not beyond the toxic levels.The reactions to the pharmaceutical substances gained in this manner were recorded in minute detail.

This list of ‘remedy symptoms’, incurred on relatively healthy organisms, having been repeatedly ascertained, together with the toxological symptoms of various substances, known from existing medical writings, were combined to form a valuable part of the homeopathic pharmacology, otherwise known as the ‘Materia Medica’.

On the basis of the law of similars and pharmacological provings on healthy individuals, Hahnemann proceeded to develop an entire medical system.

He also published the book Chronic diseases in which he maintained that all chronic diseases are a result of the suppression either of skin diseases of the skin or syphilis or gonorrhea. That is to say that if these three categories of diseases are treated wrongly, they remain within the organism in a suppressed state, in a changed form and finally they reappear as various kinds of chronic diseases.

Hahnemann named these three initial forms of disease the miasms of Psora, Syphilis and Sycosis (meaning gonorrhea). Hahnemann was heavily criticised for his theories, even by some of his supporters, who while declaring these ideas useless nevertheless continued to practise homeopathy. Some of the essential points of homeopathy, which complete the ‘law of similars’ are:

1. That there are no diseases, but only patients.

2. That the appropriate remedy must match every one of the patient’s symptoms, and not only the disease.

3. That the quantity of remedy required for a cure to be successful is infinitely small, on the condition that the ‘true’ remedy has been found.

4. That the symptoms of a disease, during the course of treatment, disappear in the reverse order of their appearance.

Individualising each case

An important idea introduced by Hahnemann and which causes homeopathy to differ from conventional medicine is individualising. He maintained that there are no diseases, but only patients, and therefore a treatment must aim at treating the individual rather than the disease. According to this idea, ten patients suffering from the same ‘disease’, for example epilepsy, will probably each require a separate pharmaceutical substance, unless the symptoms of two or three of them are identical down to the last detail, a rare but not impossible situation.

The defence system

The complex mechanism of the organism’s defence system was named ‘vital force’ by Hahnemann. This phrase expresses the idea that there is a form of ‘energy’ behind the defence mechanism, a ‘power with mind’ which directs and coordinates the organism’s reactions. The logic behind this idea, according to homeopaths, lies in the understanding that symptoms are nothing more than the expressions of the human organism’s defence mechanism. For example, the body’s temperature increases – fever – in order to suppress the disease factor, bacteria, viruses, fungi etc… therefore the organism needs support in its therapeutic efforts, rather than a suppression of the fever. Homeopaths believe that, by giving a remedy, which is capable of causing symptoms in a healthy individual that are identical to those present in the patient, they are in reality stimulating and providing support for the defence mechanism in its attempt to rid itself of the disease.

Homeopaths maintain that the true reason for disease is not infection by a disease element but rather the innate weakness of the defence mechanism and its inability to face up to the invasion and to neutralise it. By strengthening the defence mechanism, without killing the microbes, homeopaths believe that they can to a greater extent influence the return to health of the diseased organism.

Opponents of homeopathy maintain that homeopaths do not look for the true cause of an illness, and that they only treat the symptoms. Homeopaths consider that by strengthening the defence mechanism, they essentially enter more deeply into the true cause of the organism’s imbalance, which is the weakness of the constitution and its inability to fight the disease unaided. Moreover they maintain that in most cases of chronic disease the cause of the illness is unknown and therefore hard to locate and eradicate with conventional medicines.

The conflict between homeopathic and conventional doctors

     The greatest conflict concerning homeopathy lies in the quantities of the doses used, which according to conventional doctors is infinitely small and is not justified in having any action at all.

          Homeopaths on the other hand maintain that their experience shows that high potencies (multiple dilutions) have even better therapeutic results and that the action of the pharmaceutical substance is probably due to the appearance of a ‘new refined form of energy’ that appears during the potentization procedure. Therefore we are not dealing with a remedy that acts at a chemical level, but rather at an ‘energetic’ level .

Over the last few years the conflict has grown, including between scientists from other branches that are beginning to show an interest in the area of homeopathy. Some people believe that the therapeutic force is transmitted with the potentizing procedure and the powerful succussion of the remedies to the water molecules which thus contain the ‘memory’ of a new energetic state. Others attempt to explain the action of homeopathic medicines by the ‘small clusters’ theory. Yet others maintain that their action is due to the change in structure of the water molecules under the influence of the substance with which it comes into substance, as well as to the powerful succussions to which the dilution is subjected, and therefore that the dilution of the water becomes organically active.

DRUGS-

- Condurango is used to treat stomach cancer Belladonna - menstrual pain, PMS, exhaustion

- Conium is used when there are hard glands that feel like a stone. Cancer metastasizes to the lymph glands because the lymph glands act like a filter. You should give Conium wherever you see a hard gland.

- Cadmium Sulph. is handy for stomach cancer. Its keynote is coffee colored vomit, which is blood from the stomach. Use it for black vomit as well.
- These are very chilly people, as chilly as ars. alb., and have great prostration,  

- Euphorbium is for skin cancer and late stage breast cancer, when the cancer develops an open wound. Euphorbium has burning pains. It can also be used in bone pains. It is better from motion and cold applications.

- If you see sarcomas with open wounds, skin infection, or gangrene use Ars. Iod., in the 3X-6X potency without plussing morning and evening.

- It can also control infections in diabetes. Secale is also useful in improving the circulation. Hydrastis is good all over. One symptom of colon cancer is alternating constipation and diarrhea. It is treatable by Hydrastis. It can also be used in a pre-cancerous state with dyspepsia. Hydrastis has sinking empty hunger with loathing of food. Even the sweat of Hydrastis can be yellow

- Kali salts have an affinity for the mucous membranes. All Kalis are chilly except for Kali Sulph. and Iod. Whenever something is wrong with the mucous membranes and you can't figure it out, think of Kali. Polyps of the nasal passages suggest Kali. Kali can have debility, and a 2 to 5 AM aggravation.

- Tumor arising from the kidney calls for a kali salt. Mag. phos. is the remedy for spasmodic pains. It's a fantastic remedy for dysmenorrhea. It acts better in hot water.
- Aconite, opium, phosphorus, and veratrum album are the remedies for fears. Late stage cancer has many fears. Neuralgic pains suggest aconite.

- Plumbum met. and Iod. have geat affinities for colic and constipation. This is the contradiction of Plumbum. The main clue is its affinity for nervous system. It is the main remedy for brain tumors. It is also useful for demylenizations.

- Any space occupying lesion of the brain suggests Plumbum Iodatum.

- Drugs useful in breast cancer are Pulsatilla, Phos., Nat. Mur., Sepia, Staph., Conium. Scirrhinum, Thuja, Baryta Iod., Baryta Brom., Sang., Ars. Iod., and Euphorbium.
- Scirrhinum closely resembles phosphorous. Patients needing it are thin built, have a desire for chilled drinks, and are chilly and fearful. It has glandular affections and hemorrhoids.
- The glands are stony hard, just as in Conium. It is also useful for myeloid leukemia.

- Symphytum usually used for promoting healing of fractures and injuries to the orbit of the eye. I use it for tumors arising from bones, sarcomas and osteoclaustomas. Textbooks say it is useful for exostosis. Terebintha has an affinity for the bladder. It is used in bladder carcinoma.

- Thuja particularly is suited for cancer of rectum and breast.

- Scrophularia nosoda is used for skin diseases, Hodgkins disease, Non-Hodgkins lymphoma, breast cancer, tubercular adenitis, or tubercular glands anywhere. The glands are not hard, which fits in with symptoms of Hodgkins. It can also be used for asthma, hemorrhoids, vertigo or drowsiness (like Nux Moschata.)

- Strontium Carb. acts on malignant neoplasms of the bone.

- The first confusion about Carcinosin is that it cannot be used in a cancer case, only for a patient with a family history of cancer. My work has proved to the contrary.
